Creating Healthy Schools

Healthy habits contribute to lower risk of disease, improved emotional health, increased focus and learning, and other qualities that contribute to a better life. So how do we ensure kids are learning healthy habits? By creating healthy schools.


Kindergarten Requirements

1. Birth Certificate

2. Social Security Number

3. Kentucky Immunization Certificate

4. Preventative Health Care Examination – within one year prior to admission to school

5. Vision examination completed by optometrist or ophthalmologist – no later than January 1 of the first year that a 3, 4, 5, or 6 year old enrolls into public school/preschool/Head Start.

6. Dental exam/screening – no later than January 1 of the first year that a 5 or 6 year old enrolls into public school

ALL STUDENTS: Must have an up to date (Current) Kentucky Immunization Certificate on file.


Students Entering 6th Grade

1. Kentucky Immunization Certificate – Tdap and Meningococcal required

2. Second Preventative Health Examination – within one year prior to entering 6th grade

ALL STUDENTS: Must have an up to date (Current) Kentucky Immunization Certificate on file.


High School Students

1. Students aged 16 years or older, must show proof of having received two doses of Meningococcal vaccine.
